Medicare
We accept most major insurance plans, but please confirm we are on your plan at the time the appointment is made. Co-pays are due at the time of service. We will file insurance for you and bill you for any approved balance after we have been paid by your insurance. If you have an HMO/PCP, it is your responsibility to obtsin any authorizations required for us to see you and we must have authorization prior to your visit.
We currently do not accept medicaid.
